We moved a service on EKS from a database password to RDS IAM authentication. It worked at startup, then new connections failed with PAM authentication failed 15 minutes later: the app generated one token at boot and used it as the pool's password. How we generate tokens per connection.

Every few hours our FastAPI service started failing with QueuePool limit of size 5 overflow 10 reached, then recovered on its own. Two code paths opened SQLAlchemy sessions nobody closed, and only the garbage collector gave the connections back. Full repro, fix and pool monitoring inside.
